The following are the costumes available to Phase 4 in the 2013 title, Dead or Alive 5 Ultimate.

At the start of the game, Phase 4 has six different costumes, with two being available from the start, and the remaining four being unlocked through either Arcade Mode, Time Attack Mode, or Survival Mode.

Phase 4's costumes are unlocked in the order listed, regardless of the difficulty and mode played. For the tag team option, both characters will unlock a costume, and all of the costumes can be unlocked even from courses that were already cleared.

With the release of the downloadable costumes, Phase 4 was given nine different DLC costumes, adding her total up to fifteen costumes.

NOTE: Due to the Dead or Alive 5 Last Round update, this costume list is now obsolete.

Phase-4 Gym Costume Costume 7 - Grey-and-pink track hoodie with a "P" on the front and "4" on the back, and a Japanese school gym uniform which includes navy blue athletic bloomers, a pair of socks, and white sneakers.

She also has a name tag on her chest with the text "PHASE 4" written across it.

Phase-4 C8 Costume 8 - Dressed as Santa's helper.

The costume consists of a black minidress with white fur, matching gloves and boots, a fur choker with a small bell, and a Santa hat.

DOA5U halloween vol2 phase 4 costume 14 Costume 14 - Dressed as the Grim Reaper.

The costume consists of black bodysuit with a rib cage, a pair of tattered pantyhose, high heeled boots with wings on the back, black-and-green sleeves, a tattered, hooded vest, and a pair of belts made from bones.
She also has a scythe secured to her back.

DOA5U costume xmas 2014 Phase-4 15 Costume 15 - Dressed as a reindeer.

The costume consists of a hooded crop jacket with fur and a pair of antlers and reindeer ears on the hood, a matching miniskirt with stars, a pair of red-and-white striped socks, leather boots with pom-poms, and a choker with a bell and holly leaves.
